CASE STUDY 1 – SUSAN

Susan Smith is a Registered Nurse who has been practising in a metropolitan mental health team for a number of years. She has decided to move to Littletown, a small Aboriginal community in a remote area of the Kimberly, to participate in the development of their youth suicide strategy.

Susan is enthusiastic about her new role however on arrival she encounters many issues that she has not encountered before. Most of her clients are Aboriginal and though they seem to be there by choice, many of her clients will not look her in the eye and seem reluctant to talk. While many of the women seem introverted and say very little in Susan’s presence, she later see’s them together talking and behaving in a loud and extroverted way outside the clinic.

The men have a tendency to talk to Susan as though she is a child and are determined to talk about their physical health and their families not about themselves personally. Susan also senses a subtle sense of segregation within the community between the Aboriginal and non-Aboriginal people that live there. She feels unsure how she fits into the environment.

After about six weeks of working with the youth suicide strategy team Susan feels out of her depth, finding the work challenging and confusing. Many of her contributions are ignored and she is frequently told that “suicide is different for Blackfellas” contributing to her view that there is a gender hierarchy within the taskforce.

Reflection results in Susan questioning her own professional competence and how she applies her knowledge and skills when treating new clients. This causes Susan to become angry and feel isolated. She decides to discuss her concerns with Daisy, a colleague on the committee and an Aboriginal health worker at the health centre. Daisy seems to be very close to the locals and agrees to mentor Susan.
