Select one Healthy People 2020 Leading Health Indicator (LHI) that relates to your participating family’s community.

Assess the health services available in your participating family’s community.

Examine 4–6 community and public health agencies such as the following:

Public and private social service agencies
Schools
Conduct phone calls, interviews, and Internet research to evaluate the services provided by these agencies. Answer the following for each public health agency:

• Any community partnerships, coalitions, or planning groups
• The main health services and resources provided by each agency
• Each agency’s goals for achieving a healthy community
• How the agency addresses primary, secondary, and tertiary prevention
• How each agency meets the health needs of vulnerable populations within the community
