Select two or more of the following chronic health conditions for your review:
a. Hypertension
b. Type 2 diabetes mellitus
c. Chronic obstructive pulmonary disease
d. Chronic kidney disease
e. Cancer
f. Depression
g. Another chronic health condition (discuss with Unit Coordinator)
Your essay should conform to the following format:
• Introduction
In 100-150 words, provide a brief introduction of what you intend to discuss in the essay
• Explore why these chronic health conditions co-exist (500 words)
Outline the prevalence in Australia (or if you are international student, your home country) and examine the
inter-relationship between two or more chronic conditions
• Nursing interventions (650 words)
Critically analyse the two-three nursing interventions required for these co-morbid chronic health conditions.
This should include critical review of the evidence underpinning these interventions (e.g. research, evidencebased
reviews [JBI, Cochrane, etc], best practice guidelines).
• Conclusion
In 100-150 words, summarise the content of the essay, emphasising the most significant factors that must be
considered in the nursing management of the co-morbid chronic health conditions you have discussed.
