The focus of this assignment is to complete an assessment and develop a plan that can be implemented over a six-week period in a community setting.
The implementation of your project will occur in N430. Please review the requirements for the final project at the start of the course as you will be working on this over the entire course period. Clarify any issues with the professor prior to developing the project.

The paper is to be 7-10 pages in APA format, and include a minimum of 5 scholarly references.

?Diagnose – Complete your diagnosis by week 4 and include the following points about your diagnosis in your paper due week 8: ?Develop a community health nursing diagnosis based upon epidemiologic data.
