5. Written behavioral objectives, copy of pre/ posttest, content outline and student evaluation tool must be submitted via the assignment tool. ( Pre/Posttest should be based upon the objec¬tives)

6. Community health education project must be completed and submitted on or before the designated due date or a zero will be recorded.
