There are two parts to this assignment including:
1. The Case Study (2500 words).
a. Detailed assessment and description of current presenting symptoms, and problem behaviours, including sufficient information to argue for a specific diagnostic formulation.
b. An analysis of the context within which the problem arose, including premorbid factors, patient vulnerability, family and psychological factors, and life events.
c. A discussion of current problems in living, associated stress, and an explanation of how this may influence the course of the illness.
2. The Logbook (250 words)
The purpose of the logbook is to record details of the process and content of casework relating to issues of diagnosis and assessment with the client.
The clients confidentiality must be ensured at all times and NO identifying data should be included in this log. It should be a record of client contact, interviews, assessment and interventions relating to the client with whom you are working. Also include a record of any team meetings and supervision sessions relating to your client.
The content of the logbook should focus on issues relating to diagnosis and assessment.
You can choose to focus on one client or several. If you are currently not active in casework, you will need to obtain cooperation from a willing and informed client (ensuring that the appropriate protocol for the relevant mental health service is adhered to) who will provide the basis for your case study.
The Logbook should be in the form of a diary. It is suggested that a notebook is kept from the start of the case and submitted as you have used it on the due date. It is not necessary to rewrite the logbook for presentation. It should reflect the fact that it has been used as a day-to-day log of your activities, reflections and conclusions regarding your involvement in the case study.
